Rules for nominations:

1. All nominators are required to purchase/register at least (2) two tickets to the event if your nominee is chosen to be honored. (Must be purchased by 2/15/19)

2. Your nominee must be present at the event in Marietta,GA.

3. You as the nominator must be willing to be filmed/photographed for media purposes.

4. The nominee must be living and not deceased.

5. The nominee has to be willing to be filmed/photographed and come up on stage to be recognized.

6. You must fill out the entire form.

7. You must submit at least 7 sentences of why you believe your nominee should be accepted and honored.
